For your presentation proposal to be considered, please complete all sections of this form.

First Speaker

Will be used a primary point of contact.

Only the first two speakers will receive complimentary hotel rooms and travel reimbursement. Speakers not receiving complimentary rooms or speakers who would like extra nights, must pay for their rooms at the conference rate. For speakers paying for their rooms, those rooms will be held for 30 days after this agreement is signed so those speakers can make their own room reservations.
